Community Health Nurse, Sullivan County

Sullivan County Public Health Services is seeking to fill the following position:
COMMUNITY HEALTH NURSE (PUBLIC HEALTH)

This is a professional nursing position in the local health unit responsible for providing nursing assessment and education to clients in the home as well as in the clinical setting. Work is performed under the general direction of the Supervising Public Health Nurse and/or Supervising Community Health Nurse and with the guidance of the Public Health Nurse. This position differs from Registered Professional Nurse in the level of duties and responsibilities. The Community Health Nurse is a resource to the Registered Professional Nurse. The position requires independence in the field and substantial specialized judgment and skills. The incumbent is also responsible for education programs and presentations to community groups as assigned. Supervision may be exercised over the work of Licensed Practical Nurses and Home Health Aides.

MINIMUM QUALIFICATIONS:
Licensure and current registration to practice as a Registered Professional Nurse in New York State and either:
(A) Baccalaureate Degree in Nursing from a regionally accredited or New York State registered college or university; or
(B) Graduation from a regionally accredited college or university or one accredited by the New York State Board of Regents to grant degrees with a Bachelor’s Degree or higher in health or the human services field; or
(C) Three (3) years clinical experience in home care, a hospital, community based outpatient health care or residential health care setting.

Annual Salary – $72,231.00
